Key responsibilities
  • 01one counseling and group interventions to support students experiencing emotional distress, trauma, or behavioral challenges. Serve as a liaison between students, families, school staff, and community
  • 02Advocate for clients or patients to resolve crises.
  • 03Educate clients about end-of-life symptoms and options to assist them in making informed decisions.
  • 04Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs.
  • 05Refer patient, client, or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
  • 06Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy.
  • 07Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.
  • 08Identify environmental impediments to client or patient progress through interviews and review of patient records.
  • 09Counsel clients and patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
  • 10Plan discharge from care facility to home or other care facility.
  • 11Organize support groups or counsel family members to assist them in understanding, dealing with, and supporting the client or patient.
  • 12Modify treatment plans to comply with changes in clients' status.
Required qualifications
  • keeping skills. Minimum 2
  • 3 years of experience working in a school or child
  • 5 years of experience working in educational or community
  • based settings, with a focus on student support services. Proven experience in crisis intervention, case management, and trauma
  • paced educational environment.
Preferred qualifications
  • centered environment preferred.
  • Bachelor’s degree in Social Work or a related field; Master’s degree is highly preferred. Valid professional license or certification in social work (e.g., LCSW, MSW, or equivalent). 4
